• List of questions (questionnaire) that is filled out in front of you at our orientation meeting.

• From time to time, we may also use your information to contact you directly so we can better customize our website according to your interests.

• You may at anytime ask for the questionnaire back as this is the only copy and will not be copied or saved on any computers at any time.
